An employee is required to report any work-related illness or injury to their supervisor immediately. If the employee is seriously injured, he/she should seek immediate medical treatment.
In case of serious or fatal accident or hospitalization, notify EH&S immediately at (206) 543-7262. After hours contact the UW Police Department (UWPD) Dispatch by dialing (206) 685-UWPD (8973), in order for the EH&S Staff On Call to be notified. EH&S must report to the L&I Division of Occupational Safety & Health immediately after an incident that causes a fatal (or possibly fatal) injury, or that causes injury requiring in-patient hospitalization of any employee.
Employees are required to fill out an accident report as an internal record of the incident within 24 hours of the event or claim.
Employees working in Harborview Medical Center, UW Medical Center, and other healthcare delivery locations should complete the internal accident report used by their facility (PSN).
All other employees may complete the accident report online at the University of Washington’s Environmental Health & Safety Website.

These internal reports are specific to the University and will not initiate a workers’ comp claim.
