What is a claim? A claim is a written request for compensation based on an allegation of negligence. When we receive a claim we investigate and provide a decision regarding the University’s liability. Claim Services staff cannot provide advice about filing a claim. You may wish to seek legal assistance. This information is provided for access to the claims process, and does not constitute legal advice.

If you would like to file a claim regarding a vehicle accident, please complete the UW Vehicle Accident Claim Form.

If you would like to file a claim regarding an incident other than a vehicle accident, please complete the UW Liability Claim Form.

In addition to one of the above forms, please fill out the CMS Affidavit Form.

What is the CMS Affidavit form? If you are a Medicare beneficiary and receive a settlement over $750 from the UW for a claim involving medical care, we are required to report the settlement to the Center for Medicare and Medicaid Services (CMS). The CMS Affidavit form helps us comply with Medicare law.
